The visit has four parts and they happen in order. First the history, which is longer than you expect, because what makes the pain better and worse is usually more diagnostic than any single test. Then the examination: movement, strength, joint range, neurological screening where the symptoms call for it, and the specific provocation tests that separate the plausible causes from each other. Then a working diagnosis said out loud in plain language, including what we are not sure about. Then treatment, the same day, plus a written plan you leave with.
You should leave the first visit able to answer three things: what is most likely going on, roughly how long it should take, and what you are doing between now and the next visit. If a clinic cannot give you those after an hour, the hour was not an evaluation.
Most mechanical problems respond inside four to six weeks of structured work. You get an honest range at the evaluation rather than an open ended plan, and the home programme gets shorter as you improve, not longer.
Hands on work has a specific job inside that plan. It buys range, settles a guarding muscle, and makes the next twenty minutes of loading possible. It is the opener, not the treatment. Where a specific tool is indicated, it gets used and it is priced on its own page: dry needling, spinal adjustments, manual therapy, or shockwave for a tendon that has already stalled. None of them is added by default and none of them replaces the loading.
What you are actually paying for across those visits is progression: load climbing as tolerance climbs, the exercises looking less like rehab and more like training, and the point where you no longer need someone in the room.
