SMS Messaging Program
These SMS Terms & Conditions govern the text messaging program operated by Merritt Mobile Therapy, PLLC dba Physio Plus Physical Therapy (“Physio Plus,” “we,” “us,” or “our”). By opting in to receive text messages from Physio Plus, you agree to these terms.
Our SMS program is operated in compliance with the Telephone Consumer Protection Act (TCPA) and the guidelines of The Campaign Registry (TCR) and CTIA Messaging Principles and Best Practices.
Program Description
Merritt Mobile Therapy, PLLC dba Physio Plus Physical Therapy uses SMS text messaging to communicate with patients and prospective patients regarding:
- Appointment reminders and confirmations
- Scheduling changes and cancellation notices
- Post-visit care instructions and home exercise reminders
- Billing and payment notifications
- Practice updates relevant to your care
- Responses to inquiries submitted via our website or intake forms
Messages are sent from (903) 492-5215 or a shortcode associated with Merritt Mobile Therapy, PLLC dba Physio Plus Physical Therapy.
How to Opt In
You may enroll in our SMS program by any of the following methods:
- Online Form: Checking the SMS consent checkbox on our online scheduling or patient intake form at physioplustx.com
- In-Office: Providing verbal or written consent on our intake paperwork at your first visit
- Keyword: Texting START to (903) 492-5215
By opting in, you confirm you are the account holder or have the account holder’s permission to enroll the phone number provided, and you consent to receive automated text messages from Merritt Mobile Therapy, PLLC dba Physio Plus Physical Therapy at that number.
Message Frequency
Message frequency varies depending on your appointment schedule and care status. Patients actively receiving care typically receive 2 to 6 messages per month. This includes appointment reminders leading up to each visit, post-visit follow-ups, and any scheduling or billing notifications relevant to your care.
We will not send promotional or marketing messages under this SMS program. All messages relate directly to your care, appointments, or account.
Message and data rates may apply. Standard carrier messaging and data rates apply to all text messages sent and received. Contact your wireless provider for details on your plan’s messaging rates.
How to Opt Out
You may cancel your enrollment in our SMS program at any time:
- Reply STOP to any text message from Physio Plus
- Contact our office at (903) 492-5215 and request to be removed
After you reply STOP, you will receive one final confirmation message. No further messages will be sent unless you re-enroll by replying START or contacting our office.
Privacy & Data Use
Your phone number and SMS opt-in information are used solely to deliver the messages described in these Terms. No mobile information (including SMS opt-in data and phone numbers) will be shared with third parties or affiliates for marketing or promotional purposes.
Information shared via text message is handled in accordance with our Privacy Policy. Health information shared via SMS is additionally subject to HIPAA protections.
Carrier Information
Our SMS messaging service is supported by all major US wireless carriers including AT&T, Verizon, T-Mobile, and regional carriers. Carriers are not liable for delayed or undelivered messages.
Modifications & Termination
Merritt Mobile Therapy, PLLC dba Physio Plus Physical Therapy reserves the right to modify or terminate the SMS messaging program at any time, with or without notice. We may also update these Terms at any time. Continued enrollment after changes are posted constitutes your acceptance of the updated Terms.
